How to Renew

Complete Online Renewal Form
Log into your provider portal account and complete the renewal form online. Please review all information and fill out completely so the database can be updated and current.

Verify CE Requirements
Ensure you have completed all continuing education requirements for the renewal period.

Submit Payment & Form
Sign the form and mail it along with your $200.00 renewal fee to the Board office.
