Basic Requirements
Optometrists wishing to obtain a license to practice optometry in North Dakota may download an application form on this website, and send it to the Executive Director of the Board along with the license application fee of $200.00.
Education & Examination
- • Be of legal age
- • High school education or equivalent
- • Graduate of accredited optometry school
- • Pass all three parts of NBEO tests
- • Pass North Dakota Jurisprudence Examination

- • Certified copies of NBEO scores
- • Official optometry transcripts
- • Certified birth certificate
- • Two letters of recommendation
- • Current CPR certificate

License by Endorsement
Under certain circumstances, an applicant may secure a license to practice optometry in North Dakota without taking the required examinations. The applicant must properly and completely fill out all of the necessary application documents as established by the Secretary of the Board, including payment of all application and license fees.
Eligibility Requirements
- • Minimum 5 years practice in another state
- • License from state with equivalent requirements
- • Reciprocal privileges with North Dakota
- • Current license in good standing

- • Pass North Dakota Jurisprudence test
- • Letter from current state board
- • No disciplinary actions in last 5 years
- • Current CPR certificate
Important: North Dakota law allows the Board to give a practical examination to any applicant, if it is deemed necessary by the Board.
