Basic Requirements

Optometrists wishing to obtain a license to practice optometry in North Dakota may download an application form on this website, and send it to the Executive Director of the Board along with the license application fee of $200.00.

Education & Examination

  • • High school education or equivalent
  • Graduate of accredited optometry school
  • • Pass all three parts of NBEO tests
  • Pass North Dakota Jurisprudence Examination

Under certain circumstances, an applicant may secure a license to practice optometry in North Dakota without taking the required examinations. The applicant must properly and completely fill out all of the necessary application documents as established by the Secretary of the Board, including payment of all application and license fees.

  • • License from state with equivalent requirements
  • • Reciprocal privileges with North Dakota
  • • Current license in good standing
